Vital Signs

Common adult resting ranges: Pulse 60–100 bpm • Respirations 12–20/min • SpO₂ commonly 95–100% • Normal BP is generally less than 120/80 mmHg. Temperature varies by route and clinical situation.

Visual Acuity

Remember: OD = right eye • OS = left eye • OU = both eyes. Always document whether glasses or contacts were worn during testing. 20/20 is commonly considered normal distance visual acuity.

Orthostatic BP: Record BP, pulse, and symptoms with position changes. A decrease of ≥20 mmHg systolic or ≥10 mmHg diastolic within 3 minutes of standing is commonly used to define orthostatic hypotension. Follow instructor/facility procedure and protect the patient from falls if dizziness or weakness occurs.
